Many people assume that the pain must be caused by something “wrong” inside the knee — but that’s not always true.


MRI “Abnormalities” Are Common — Even Without Pain

A 2020 study by Horga et al. looked at 230 people with no knee pain at all. Surprisingly, 97% of them showed “abnormalities” on MRI.
Here’s what the scans revealed:

  • 30% had meniscus tears

  • 62% had cartilage changes

  • 52% had bone marrow edema

  • 46% had tendon abnormalities

  • 38% had ligament changes

  • 26% had pre-patellar bursitis

These findings show that structural changes in the knee are often normal and not necessarily the cause of pain.

So if you’ve had an MRI or X-ray showing one of these “abnormalities,” don’t panic — the issue may not be in the structure itself. Often, pain comes from:

  • Muscle weakness or imbalance

  • Training or movement pattern errors

  • Poor tissue health

  • A sudden spike in activity levels
